Federal

Apply for a Federal Employer ID Number The IRS has created a place on the web that allows you to apply for an EIN on line. Fill out the form and receive your new EIN immediately.

W-4 This form is used to report an employee’s federal filing status.

I-9 This form is used to verify a worker’s right to work in the US.

Note: This form has been modified as of 3/8/13. As of April, 2013, employers must use the new form as the old form is no longer acceptable. Failure to comply may result in penalties.

E-Verify The US Citizenship and Immigration Services information site on use of E-Verify.

Note: Remember, you must have a W-4 and I-9 on file for each of your employees. Failure to comply with these regulations may result in fines of up to $10,000.00 per occurrence.

WOTC 8850 Federal tax credit available to employers who hire and retain veterans and individuals from other target groups with significant barriers to employment.

SS-8 If you are wondering if you should treat an individual who is going to work for you as an employee or an independent contractor, you can get input from the IRS using this form.

W-9 Use this form to request a tax payer ID number for an independent contractor (IC) you have hired. You will need this TIN for year-end filings.

8822 If your mailing address has changed, you need to notify the IRS to ensure that you receive any IRS refund or correspondence. You may correct the address legibly on the mailing label that came with your tax package, or write the new address on your return when you file. When your return is processed, the IRS will update your records. Or you can use the form 8822.

OSHA 300 Log This form is required to keep track of your employee’s work related injuries.

Georgia

G-4 If you are a Georgia employer, you need a G-4 for each employee.

North Carolina

Registration Application for State Withholding ID Number

On-Line Registration Application for NC Unemployment Account It is recommended that you do not apply for an NC UI account number until you have employees physically working for you.

NC-4 If you are a North Carolina employer, you need a copy of a NC-4 for each employee.

Tennessee

Tennessee Application for Employer Number (State Unemployment Account)

Tennessee C20 Form In Tennessee, you are required to submit a First Report of Injury form when an employee is injured on the job. If you need a copy of this form, please click on the above link.

Tennessee C42 Form In Tennessee, you must provide an injured employee a panel of doctors for that employee to choose from to provide care for their injury. This is done on the C42 form.

Tennessee Separation Notice Tennessee requires all employers to furnish a Separation Notice to each separated employee within 24 hours of separation from employment. You do not need to issue one to an employee who has worked less than one week. The form streamlines the administrative processing of unemployment claims.

Tennessee Notice of Corporate Officer to Employer of Election NOT to Accept Provisions of Worker’s Compensation Act. This form is used for officers of the company to opt out of workers comp coverage.

Note: The state and thus workers comp insurance companies are sticklers about making sure you have this form filed with the state. If you do not have this form filed with the state, you will pay workers comp ins premiums on officers even though you may not want to. We suggest you get further information on this topic from your workers comp agent.

Tennessee Drug Free Work Place The Tennessee Drug Free Workplace Program is designed to increase productivity for Tennessee employers and promote safe work sites for employees by promoting drug and alcohol-free workplaces.
